How to retrieve your completed registration form online

If you want to retrieve your completed registration form for printing purposes, or if you just want to save a PDF copy, follow this procedure:

Step 1: Go to www.cdsa.org.ph and click "Retrieve Completed Form" in the Enrollment Menu.

Step 2: Type your ID Number in the box and press Enter.




Step 3 and 4: You will receive an e-mail providing you a link for the form. The link expires after one hour for security purposes.

Step 5 to 7: You can print or save your completed form from here.
